Entry condition report – general tenancies (Form 1a)
The Entry condition report – general tenancies (Form 1a) records the condition of the property (e.g. houses, units, houseboats) at the start of the tenancy.
The Entry condition report (Form 1b - moveable dwellings) records the condition of the premises (e.g. caravan, caravan site, mobile home) at the start of the tenancy.

Bond lodgement (Form 2)
The Bond lodgement is used when lodging either a full or part-payment of rental bond money (for Queensland tenancies) with the Residential Tenancies Authority (RTA).

Change of rental property (Form 3)
The Change of rental property (Form 3) transfers bond money from one property to another and only allows transfers where the lessor, agent or manager/provider remains the same.

Refund of rental bond (Form 4)
The Refund of rental bond (Form 4) is used to request a rental bond refund from the Residential Tenancies Authority (RTA). This form should be completed when the tenancy agreement has ended.

Bond refund for persons experiencing domestic and family violence (Form 4a)
Tenants/residents and managing parties can use this form to request a rental bond refund for their bond contribution or a tenant/resident’s bond contribution once a tenant/resident’s interest in a tenancy/residency has ended on the grounds of experiencing domestic and family violence.

Continuing interest notice
The Continuing interest notice has been developed to support property managers/owners, agents or managers/providers to fulfil their obligations under the Residential Tenancies and Rooming Accommodation Act 2008.

Change of property manager/owner (Form 5)
The Change of property manager/owner (Form 5) is used to notify the RTA there has been a change of lessor, agent or manager/provider in an ongoing tenancy. It must be completed and signed by the previous and new lessor, agent or manager/provider.

The Change of bond contributors (Form 6) is used when there is a change of bond ownership in an ongoing tenancy.

The Signature record (Form 8) is used to register with the RTA all names and signatures of people authorised to sign RTA forms.

The Entry notice (Form 9) is used to give notice to tenants about an entry by the lessor/agent.

Notice of lessor's intention to sell premises (Form 10)
The Notice of lessor's intention to sell premises (Form 10) is given to the tenant/s to advise them of the intended sale of the rental property.

Notice to remedy breach (Form 11)
The Notice to remedy breach (Form 11) is used during the tenancy where either the tenants or the lessor/agent claim that there has been a 'breach' of one (or a number) of terms of the residential tenancy agreement.

The Notice to leave (Form 12) is used when the lessor/agent is giving notice to the tenant/s to vacate the property. Correct notice periods must always be given.

The Notice of intention to leave (Form 13) is used when the tenant/s are giving notice to the lessor/agent that they wish to vacate the premises by a certain date.

The Exit condition report (Form 14a) records the condition of the rental premises (e.g. house, townhouse, unit, houseboat) at the end of the general tenancy.
The Exit condition report (Form 14b) records the condition of the rental premises (e.g. caravan, caravan site, mobile home, rented manufactured home) at the end of the moveable dwelling tenancy.

The purpose of the Dispute resolution request (Form 16) is to advise the RTA that there is a dispute and to request assistance in resolving the dispute.

Notice to vacate from mortgagee to tenants (Form 19)
The purpose of the Notice to vacate from mortgagee to tenant (Form 19) is to inform the tenant/s of a rental property that the mortgagee (or appointed person) will be taking possession of the premises and ending the tenancy agreement.

Notice ending tenancy interest (domestic and family violence) (Form 20)
A tenant completes the Notice ending tenancy interest (domestic and family violence) when they believe they can no longer safely occupy the premises due to domestic and family violence and wish to end their interest in their current tenancy and vacate the premises.

Domestic and family violence report
The Domestic and family violence report which must be completed and signed by an authorised professional can be provided as a type of relevant evidence to support that a tenant/resident is/has been experiencing domestic and family violence.

Request for approval to keep a pet in a rental property (Form 21)
Tenants must use this form to request approval from the property owner to keep a pet or animal in their rental property.
Tenants should complete a separate form for each pet. Property owner’s approval is not required to keep a working dog at the property.
